The process is similar to dialysis. In AML, the body makes unhealthy blood-forming cells (stem cells) that don't develop properly. So theres clearly a need. So the average age of the dogs that I transplant is four. Median survival means 50% of pets live less than that time point and 50% live longer. [00:26:50] >> James Jacobson: So a lot of these dogs obviously, probably already have local oncologists, if not general practice vets. There are only a couple of facilities in the United States or in the world that do it, both here in the United States, and we are speaking today with the worlds leading expert on this. Bone marrow transplantation in people is a standard of care and over the last 6 years it's been proven as an effective therapy for dogs with B-Cell lymphoma. You said that, you know, the people move out and basically a dog is sedated and then its exposed to the radiation for a couple hours, two days in a row. And thats the same in dogs so radiation, if we see side effects besides the obvious bone marrow issues, its gastrointestinal issues. We retrospectively reviewed medical records of dogs diagnosed with high-grade B-cell lymphoma who received an identical allogeneic HCT. In most cases your dog will be sedated or anesthetized to keep him still and calm. [00:13:39] >> Dr. Steven Suter: Yeah. Small dogs have successfully received bone marrow transplants, but this must be done with caution. Autologous peripheral blood hematopoietic cell transplantation in dogs with B-cell lymphoma. When the CBC blood values return to normal, your dog is ready to go home. A bone marrow transplant can cure some diseases and put others into remission. Dogs with serious health problems such as kidney or heart disease, or conditions that make infections more likely, such as diabetes or Cushings disease, do not qualify for the transplant procedure, as the risk would be too great. The mean number of donor CD34+ cells/kg harvested and infused into recipient dogs was 8.0 106 /kg (range: 2.08 106 /kg-2.9 107 /kg). Its not meant to take the place of the advice you receive from your dogs veterinarian. And then the drug that was used for five days prior, does that, I presume, stimulate the bone marrow, to increase the production? [00:21:30] >> James Jacobson: And you are pretty much the worlds leading expert on this at this point. In some dogs, bone marrow sampling may need to be repeated in order to see if the bone marrow is responding normally or not. The transplant procedure requires several days of preparation, starting with twice-daily injections of a drug called Neupogen, designed to drive healthy stem cells from the bone marrow into the bloodstream where they can be harvested. So we see it among small dogs, breeds, now where it usually it was a middle to large sized dog breed. [00:14:58] >> James Jacobson: But leukemia is not as common to use with this procedure as the lymphomas, as the B and T cell. [00:04:59] >> Dr. Steven Suter: Yeah. So. And many of those dogs, unfortunately, dont make it to transplant because of that. The prognosis with oral steroids alone is 1-2 months. Bone marrow transplants can be difficult for already very sick animals, who then also have to spend a prolonged period of time away from their homes. As a model for the human disease, there were two dog models there. So their white blood cell count and platelets usually go to absolute zero, which again is something that just, you know, folks in general, arent used to. So if you find four dogs, thats not a hundred percent chance. Panel A. Donor #1 is fully matched to the recipient dog at 8/8 DLA alleles. I dont know how it happened, but I am. [00:24:10] >> Dr. Steven Suter: That is pretty good. And so pick a popular breed, you know, Labs, Goldens, Boxers, et cetera, et cetera. It is used to treat certain types of lymphoma and, um, its an aggressive type of treatment that is not right for everyone. The average total hospital stay for a bone marrow transplant from start to finish is three weeks. The gut is also a rapidly dividing tissue, and thats why people who receive chemotherapy can become very ill because chemotherapy can really affect the cells of the gut, which have a very rapid turnover. [00:22:55] >> James Jacobson: And these are dogs that really, I mean, cant necessarily wait so long because they have advanced cancer. Dr. Suter, at North Carolina State, has performed bone-marrow transplants on 65 dogs, with 10 more now on the waiting list. [00:13:14] >> James Jacobson: All the way? Cure has been obtained in 33% of B celllymphomas and 19% of T cell lymphomas, although the dogs should be in clinical remission before the transplant is performed. The median disease-free interval and overall survival of the remaining three dogs was 25 days (range: 15-250 days) and 1100 days (range: 66-1902 days), respectively. We like to try allo first because the cure rates about double. So those are all issues that make this decision extremely difficult and we dont take it lightly at all. The treatment, in which dogs receive a stem cell transplant either from their own body or a blood-related donor, is only regularly offered at two places in the United States: a private practice. What Are The Side Effects Of Chemotherapy? [00:08:32] >> Dr. Steven Suter: The cost has gone up instead of down, unfortunately. But I mean, they have some reactions. Ideally, your dog will also have DNA-based blood tests run to verify that he is in molecular remission, which means that there are no detectable cancer cells.1,2,7. So sort of mirroring the chemotherapy experience. They look like, um, pretty funny, big rats for a while, but it all grows back, usually within two months or so it all grows back. Uh, the radiation is administered via a machine called a linear accelerator. So peripheral blood always has some very low number of stem cells. So its a lot of money. We have a waiting list of 16 dogs, 16 families and their dogs, all the time, which is frustrating because we cant get to them all the time. [00:21:36] >> James Jacobson: As I said, its just the two of you guys at a bar at some vet conference. Because they cover 80 to 90%, which is fantastic. Review plans carefully to determine what deductible you will need to pay and if there are any annual or lifetime caps. While the stem cells are rebuilding the bone marrow, your dog will be hospitalized and monitored closely to address any side effects and to prevent infection. And we were using bone marrow transplantation and gene therapy, which is part of my PhD, to try to treat that disease and make it go away, which actually works. Stringent safety protocols to protect the dog from exposure to infections, aggressive treatment of any possible infections with antibiotics, and refinement of radiation doses over time all work together to make this treatment as safe as possible. So, they all have to receive chemotherapy to get them to that point. We have to get blood from those dogs. 30th transplant, all done over the past two years. North Carolina State Universitys College of Veterinary Medicine in Raleigh is the first university in the world to open a canine clinical bone marrow transplant (BMT) unit. Concurrent days. [00:27:15] >> James Jacobson: And then in terms of the breeds of the dogs, obviously we know what kinds of cancer, but like, is it better for bigger dogs, or smaller dogs, or certain breeds? I see dogs with lymphoma that are one year old. Among 38 dogs treated with 8.4 Gy delivered at 4 cGy/min, 9 (24%) became long-term disease-free survivors. And so, you know, if I have an acute leukemia dog who, and he lives six months, its almost a miracle. This includes everything except intensive ICU care, blood transfusions, and additional diagnostics that may be needed in some cases. And I actually ended up becoming very interested in bone marrow transplantation because I was using that to treat a type of immunodeficiency called severe combined immunodeficiency. For most dogs, no special care is needed after they return home. We can cause these stem cells to leave the bone marrow (with a medication called Neupogen) and travel out into the peripheral blood, where we can collect them using a sophisticated apheresis machine. Bone marrow is the soft, spongy tissue found inside bones. [00:02:55] >> Dr. Steven Suter: Sure. [00:16:48] >> Dr. Steven Suter: And so thats the, kind of the line we look for is for an animal to make it past two years, once past two years, theyre good to go. And so I ended up doing a lot of bone marrow transplants during my PhD, and thats how I really learned about all the information thats out there in the research setting, as far as transplanting dogs and the use of dogs as a model. And so, you know, based on my PhD experience, I learned about bone marrow transplant, and I quickly wanted to try to do something that would help our animals after watching so many of them pass away from lymphoma and leukemia. A week later, the dog is placed on a leukaphoresis machine that harvests stem cells from the blood. And then I became an oncologist, I did a residency in oncology, and it just sort of almost accidentally all fit together into this idea that this can be done. The bone marrow transplant for dogs, also known as Hematopoietic Stem Cell Transplantation, is a time and labor intensive and expensive procedure, but 30-40% of treated dogs are completely cured (live > 2 years) of lymphoma. [00:14:11] >> Dr. Steven Suter: You can also treat dogs who have leukemia, so acute leukemias can also be treated. National Library of Medicine So 30 to 40% are cured. It can take months to find a match. So the process is the use of stem cells, which are from the bone marrow, to repopulate the bone marrow after its been killed, for lack of a better word, from the radiation. Whats the Ideal Feeding Schedule for Dogs? [00:29:56] >> Dr. Steven Suter: Thanks so much for having me. Eight of nine remaining dogs lived >4 yrs post-alloHCT, leading to a cure rate of 89%. Some people have few side effects and complications from a bone marrow transplant. On radiation day(s), your dog is put under anesthesia and receives total body radiation to hopefully kill all remaining cancer cells wherever they might be hiding in the body. So theres two types of bone marrow transplants that maybe we can get into later, but the current price for an autologous transplant where we use a dogs own stem cells is anywhere from $21,000 to $26,000. Expected survival rate for dogs that undergo a bone-marrow transplant is estimated at 30 to 50 percent. And then describe what thats like. [00:01:50] >> Dr. Steven Suter: Sure. And then they go to radiation, radiation for two days to hopefully kill all the cancer cells in their body. [00:17:06] >> James Jacobson: Okay. So usually after the radiation done, well, well go into the vault and its an intravenous infusion so well just hook, hang the bag and start giving the dogs the cells right away. The other thing I should mention about the breeds is that in the old days, back when I was in veterinary school, you know, lymphoma was a disease of middle aged to older age dogs, and it still sort of is. Antibiotics are given before and after the procedure to reduce the chances of bacteria entering the bloodstream and to help fight any infection while the immune system is suppressed. So here at NC State, we have these beautiful, positive pressure, HEPA filtered isolation wards with basically wall to ceiling glass. Overall, patients tolerate this therapy well, although this is an aggressive therapeutic option with risks from anesthesia, radiation side effects, and infection. And so when their neutrophil count, which are the cells that fight infection, go to below a thousand or so, cause we know theyre going to drop to zero, then they go into an isolation ward until those cells shoot back up again, which usually takes two to four days. We got the coasts covered. So they do spend about, usually about four days in isolation. This ensures that the veterinary staff dont have to worry too much about coat care while your dog is hospitalized (most facilities will trim nails while the dog is under anesthesia, often for free). I like that. [00:06:53] >> James Jacobson: Okay. This intense treatment is not the right fit for every dog, but can have a happy ending.Bone marrow transplants, also called tot. Following the procedure, the dog is kept at the facility in an isolation ward for about two weeks to give the immune system time to recover as the stem cells start to regenerate. So its almost half of the cure rate of what the dogs with B-cell lymphoma. [00:24:04] >> James Jacobson: Okay. [00:09:50] >> James Jacobson: So lets go back to the process because theres, it just engenders so many questions. If you are considering a bone marrow transplant for your dog, here are some things you can do to help prepare your dog: Your dog will be released from the hospital once his blood values have returned to normal or near-normal after the bone marrow transplant. Once in remission, they can have a BMT at any time afterward.. Yeah. Cancer death rates have dropped by 25% over the past 25 years, with President Biden setting a . And basically they have no immunity at that point because all of that has been destroyed through the radiation. But its not like the pet parents can come visit during that time because theyre in isolation.
